Already registered with the Heathcare Commission

Request your application forms

You will need to request your application forms from the Healthcare Commission. Call the Private & Voluntary Health (PVH) department on 0207 448 8188, or download the application forms from their website. You will also need to request the Healthcare Commission's ‘Registered Body Number' for a CRB check.

All applicants for registration of an independent healthcare service must request an enhanced check by the Criminal Records Bureau (CRB). Forms can be requested from the CRB on 0870 9090 822. Further information can be found on the CRB Website.
